5. Have you had a positive covid 19 test within the last 4 months? Y/N If so can you provide your positive results? We are asking because you can still test positive three months after you have had covid. We can’t let you work if you test positive unless you can show us that you have had a positive test within the last few months.
6..Please email a CURRENT FULL BODY SHOT AND A PHOTO FROM THE CHEST UP. No headshots, they will not be accepted(all photos must be taken within the last month). You can take a snapshot with your phone NO SUNGLASSES OR HATS ON we need to see your face. It must be a photo with only you in it.
PLEASE LABEL THE PHOTO FILE WITH YOUR FIRST AND LAST NAME.
